DO'S AND DONT'S FOR CERVICAL PATIENTS

-Do not sit for prolong period of time in stressfull postures.
-Do use firm collars while travelling.

-Do not lift heavy weights on head and neck.
-Do not turn your body,but turn your body moving feet first.
-Do turn on side while getting up from lying down.
-Do the exercise prescribed daily.
-Do use firm mattress,thin pillow or butterfly pillow.
-Do not lie flat on your stomach.
-If your occupation is prolong sitting,relax your neck regularly to relax the stressed muscles.
-Make your desk level higher to prevent neck bending.

FROZEN SHOULDER TREATMENT


INTRODUCTION- Frozen shoulder,or adhesive capsulitis(inflammation of capsule) is a condition that causes restriction and pain in the shoulder joint.The frozen shoulder causes the capsule surrounding the shoulder joint to contract and form scar tissue.
CAUSES- Most often frozen shoulder occurs with no associated injury.Some patients develop frozen shoulder after a traumatic injury to the shoulder,bt this is not the usual cause.
Risk factors for developing the frozen shoulder are: 
                                          -commonly affects age between 40-60yrs,and twice in women than men.
                                          -diabetic patients are at risk for developing frozen shoulder.
                                          -other endocrinal abnormalities like thyroid problems can also lead to this condition.
                                          -patients who sustain shoulder injury or undergo surgery at shoulder followed by prolonged immobilisation,the risk is at its highest.
DISCRIPTION- Normally shoulder joint allows more motion than any other joint in the body.When a patient develops frozen shoulder,the capsule that surrounds the shoulder joint becomes contracted.The patient form bands of scar tissue called adhesions.The contraction of the capsule and formation of adhesions cause the shoulder to become stiff and painful.
SYMPTOMS- Shoulder pain,usually a dull aching.
                       - Limited movement of the shoulder.
                       - Difficulty in overhead activites like putting on shirts,brushing hair etc..
                       -Pain when trying to sleep on affected side.
                         
                                            
STAGES OF FROZEN SHOULDER- 
              PAINFUL/FREEZING STAGE- This is the most painful stage of the frozen shoulder.The motion is restricted but the joint is not stiff.The painful stage typically lasts  6-12 weeks.
              FROZEN STAGE-During the frozen stage,the pain usually eases up but the stiffness worsens.The frozen stage can lasts 4-6months.
TREATMENT- Frozen shoulder treatment primarily consist of pain relief and physiotherapy. Most patients find relief with these simple steps,althogh the entire treatment process can take several months.
TREATMENT BY PHSIOTHERAPY- 
FOR PAIN RELIEF- 1)Moist heat- Application of moist heat to the shoulder can help to loosen the joint and provide pain relief.Moist heat can be applied by using a hot soakd washcloth on the joint for atleat 15min.
2)STRETCHING- Streching can be done immediately after removing the moist heat from the joint.It will increase the range of the shoulder and decrease the pain.
NOTE- The whole process including moist heat and stretching should be done atleast 3 times in a day.
FOR STIFF SHOULDER- In these cases the pain is almost nil,but the main problem is stiffness to the shoulder.The protocol given below is the best way to increase the range of the shoulder joint if done regularly.
1)If there is pain than donot perform vigrous exercise on shoulder.A simple thing which can be done is pendular movement.Stand at the side of table,and place the good side on table for support,and let the affected side swing like a pendulam freely.
                                 
2)Moving forward to next exercise, which name is circumduction.In this,elevate the arm till 90degrees and make circle with whole arm moving clockwise and anticlockwise turn by turn.
 

3)Hold the affected side with normal side by the hand, elevate both the arms till the maximum a patient can do upto the severe pain.The point at which the patient feel maximum pain,hold at that point and move downwards slowly as much as you can.
4)Elevate the arm 90 degrees and fold the elbow,Now place the good hand on outer surface of the affected side elbow.Give an inward force to the elbow,so that the elbow comes closer to the good side shoulder and the affected side hand should be able to touch the back on good side.

5)FINGER LADDER- i)Facing to wall and place your first 2 fingers on wall.Now moving your fingers upwards like you climbing the ladder i.e step by step.When reaches the uppermost point,hold there for 3-4 secs and than by the same way move fingers down like climbing down from ladder.Keep your elbow straight while moving down and up(it is important not to fold elbow while doing this).
ii)Do the same movement,but stand at side of wall.
6)Rotation- It is same like when we clean our back after taking bath.
Use a towel or a cloth (same length of towel),affected side holding it from down and good hand  holding overhead.Now pull the towel or cloth from the good side upwrds so dat the affectd side comes up from down.
REPEAT ALL THESE MOVEMENTS THRICE A DAY REGULARLY TO GET RELIEF FROM FROZEN SHOULDER.

OSTEOARTHRITIS OF KNEE

INTRODUCTION- The knee is the largest joint in the body.The knee supports whole of the body weight.It is vulnerable to both acute injuries and development of osteoarthritis.
DEFINITION- Osteoarthritis is a degenerative condition which primarily effects the cartialge of the joint.(cartilage is present between the joint.cartilage act as a cushion in the joints.)Due to overstress or degenerative process with age,it losses strength,which we call osteoarthritis.
CAUSES- Age of the patient is an important cause.
               - Any injury around the joint few year back.
               - Overstress on the knees.
               - Wrong position of the knees.(like squatting)
               - Obesity
SYMPTOMS- The continous pain is the primary symptom.-  pain while walking
                                                                          -  while getting up.
                                                                          - while touching the knee.
-Swelling is another symptom.
-Crepitus i.e a click sound while moving the knee.
-Deformity will be present after some years.
PRINCIPLE OF TREATMENT- If this occur once, it cannot cure with medicine.Prevention is the best method to prevent the further damage to the joint.
- The symptoms like pain and swelling can be best treated by equipments in physiotherapy like TENS and ULTRASOUND.
-To pervent further weakness and maintainance of the strength of knee, exercises are the best option.The exercise regimen is given below in detail-
Strenghtning of knee muscles- 1)Roll a towel and place it under your knee.Now press it for 5sec.Repeat it for 10times.Progresively increase the time of  hold to 10 sec.
2)-Sit at the edge of the bed or chair.Now straighten your knee.Hold it for 5 sec and then relax it.repeat it for 10 times.The patient can use wieght also to make it more vigrous.
3)- The patient shoul lie on the bed and raise his leg straight.Then hold this position for 5 sec and then place it relax on bed.To make it more vigrous increase the hold time to 10 sec.
ALL THESE EXERCISES SHOULD BE DONE 3-4 TIMES PER DAY.
-THESE EXERCISES ARE PROOVED BEST FOR THE OSTEOARTHRITIS OF THE KNEE JOINT.
PREVENTIVE MEASURES FOR AN ARTHRITIC KNEE- If it is diagnosed that the person is having osteoarthritic knee,then these preventive measures should must be adopted-
-Avoid sqautting and sitting with cross legs.
-Avoid using indian toilet seats.
-Avoid unnecesseray bending,standing and walking.
-Avoid sitting on  knees.
-Avoid unsupported climbing and getting up.
-Avoid carrying heavy objects.
-Avoid running.
-If patient is overweight or obese then he/she should loose his weight.It will help prventing overstress on the knees.
-Patient shoul have good hygienic food.
-If patient is very old he/she should use stick while walking,so that stress on the knees should be decreased.
-Never forget to do exercises mentioned above.

SPRAINS AND STRAINS

Difference between sprain and strain--
SPRAIN  - It is an injury to ligament.Ligament is a thick fibrous tissue that connects bones together.The most common injured ligaments are in knee,ankle and wrist.
WHAT CAUSES SPRAIN- The ligaments can be injured by being too far stretched from its normal position.A common sprain is an injury we often call TWISTED ANKLE.This injury is commonly occurs in activities like running,jumping and playing.
-People fall from a step or an uneven surface and roll their foot inside or outside.This stretches the ligament and causes sprain.
STRAIN-  A strain is an injury to muscle or tendon.Muscles move your skeleton in different ways.Muscles are made to stretch but if stretched too far,it can lead to strain.
-A strain can be stretched or tear in muscle.
COMMON ACTIVITIES WHICH CAUSES SPRAIN AND STRAIN- 
- Sports activities like football,volleyball,gymnastics,and many others.
 -Injury occur in normal everyday activities like slip on ice,a fall on your wrist,or jamming your finger.
SYMPTOMS OF SPRAIN AND STRAIN-
 -Pain,swelling and bruising of the affected joint.
 -The symptoms will vary with the severity of the injury- The less serious injury will cause inly pain in the joint affected.
-The more serious injury will cause severe pain and inability to move that joint.
TREATMENT- The principle of treating a sprain or strain is ''RICE''. It reffers to REST,ICE,COMPRESSION AND ELEVATION. 
REST- The part which get injured should not be use much,specially avoid the movement which cause pain.It can aggravate the problem.So avoid moving the part.
-Rest can be attain by applying a splint to immobilise that part.
 ICE - For the first 72hrs post-injury,ice the part 20min at a time every 3-4 hrs.
-Icing the part is known as cryotherapy.Ice should not apply directly to the part.The correct method of applying ice is that- take a plastic vegetable bag and put the ice cubes in that.Then aplly that bag on the injured part.
-Do not just place the ice on injured part.Massage the part with the ice in that way in which it covers all the painfull area.
-Do not ice the part more then 20 min because it can damage the tissue.
COMPRESSION- Use compression when elevating the sprain or strain in early treatment.
-Use an ACE BANDAGE,wrap the area overlapping the elastic wrap by one half of the width of the wrap.
-The wrap should be snug,but not cutting of the circulation to the part.So if your fingers and toes becomes cold and blue,then re-wrap.
ELEVATION-  Try to place your injured part higher from your heart level when lying.It can be achieved by placing a pillow under foot or hand(wherever injury occur).

FIRST AID FOR FRACTURE.

About one lac people die on the road.Many of these lives can be saved if proper first aid is given.
One out of five people has a fracture at some point of time in his life,and the maximum of these occur during childhood.
At the site of accident,how a person can identify that there is fracture to that person who met with an accident.?
The pain of fracture is very severe,even the victim wont allow anybody to touch his injured part.but if there is sprain,its pain is less intensive then a pain of fracture.
Kinds of fracture--
                    1)simple fracture-It is just when the bone gets broken and there is no external wound.

            2)compound fracture-It is one in which the bone completely breaks and come out through the skin. 

Prevention- 
     1)Do not move the injured part.Give a pressure bandage to prevent the blood loss.
The steps to be followed while first aid-First of all call for the medical help.Always have an emergency number of ambulance with you in your mobile or dairy.
Till the ambulance come you should give first aid to patient in this way-
          1)Stabilise the injured part.It can be done by using handy material like a newspaper,magazine, or  piece of wood.
     2)Tie that wood or newspaper with any colth,so that no movement is possible in any direction.
     3)Apply 2 pieces,one on inner side of leg and another on the outer side of leg.
     4)The material used for stabilising the part should be large so that whole leg is immobilise.
     5)If in case no material is found to fix the leg ,you can also tie both legs together.
     For the upper part of body-
                     -if the fracture is present at the arm or forearm tie the injured part with the same method explained above.
                    -if the fracture is present at the hand,the other hand can be use for the support of injured hand untill the proper medical help come.
In case if there is a compound fracture-
        1)Stop the bleeding.It can take the life of the victim if the fracture is of a major bone.
        2)Bleeding can be stopped by cleaning it carefully and applying a clean dressing.
        3)Do not bandage tightly over the injured part.(tight bandage can be dangerous for the part)
        4)Do not bandage the part roughly.
        5)Wrong bandaging can lead to complications like swelling or necrosis
        6)Tie the bandage patiently so that whole the wound should cover properly.

TAKE CARE OF YOUR BACK.

Most of us will have backache at some time in our lives
Backache is often caused by our everyday causes:- GARDENEING,
                                                                              CAR MAINTENANCE,
                                                                              HOUSEWORK,
                                                                              LIFTING OBJECTS,etc
Causes  -Bad posture,
              -Undue stress,
              -Injury(disc injury,vertebral injury),
              -Wrong sleeping pattern,
              -Lifting the objects in wrong way,etc..
Prevention- Improve your posture,
                                                                        Sit straight.
               - Lift the object without bending the back.
                -Sleeping on back is correct posture for backache patients.
Treatment- Consult your physiotherapist.(because treatment will start after knowing the exact cause only)
                 -For pain heat formentation can be done at home.
                 -Exercise therapy helps alott to attaing good posture.
                 -Appopriate exercises can only given by therpist.
                 -Exercises of back also prevents the further damage to spine.
COMMON EXRCISES- 1)FOR THE PEOPLE WHO HAVE TO BEND THEIR BACK FOR LONG TIMES.e.g-labour
                               -nurses
                               -doctors
                               -and all who have habit to sit slouch
                                                            1) Maintain this position for 10 sec.do the same for 10 times at morning and evening.
                                                              2)This is a self stretching exercise which helps to relax you even at your work. perform it for 5-10 times and hold the position for 5 sec.
                                                             3)place the both hands at sides of head and now lift the head.hold this position for 5-10 sec.perform it twice daily.


NOW FOR THE PATIENTS WHO HAVE TIGHTNESS IN BACK-
                                                       1)Either single knee to chest or both knees to chest and hold for 5-10 sec.
                                                      2)Raise your leg without bending your knee and hold it for 5-10 sec.

cervical spondylosis

It is 1 of the most common problem specially in youngsters..
Generally the main cause is bad posture..due to which the cervical spine shape gets abnormal and the symptoms like vertigo,pain nd dizziness start cuming..(due to entrapmnt of nerve in the vertebra)
Advice-sit straight,
             -relax the shoulders after every 15mins if heavy occupation,
             -if sitting job-then try to realx your neck by bending it back after every 15 mins
Treatment-heat formentation twice daily.
                 -exercises of neck and hold with 10 seconds.
                     -improve the posture..try to sit straight,
                     -dont bend the neck for long tyms,
                     -do shoulder shrugs repeatedly after sometime.
                     -avoid pillow under head,and use butterfly pillow specially made. 
In old age-The main reason is degeneration of the vertebras.
                 Treatment is almost same..
